Not all providers are in-network with each insurance that we accept. If you have questions please contact billing@maidenlanemedical.com
We bill your insurance plan for each visit. Your responsibility is to pay for any co-pays, deductibles or co-insurance. The amount you owe depends on the specifics of your plan. If you have any questions, we suggest you call the number on the back of your insurance card to learn more about your coverage. We accept checks, cash, debit and credit card.
Your benefit plan coverage, co-pays, deductibles, co-insurance are determined solely by your insurance.
Network Participation – IMPORTANT!

Unfortunately, insurance companies have maximized everyone’s confusion as to which plans we accept and the plans in which we do not participate. Each insurance company has anywhere between 20 and 200 different plan types for you to buy. They often come out with new ones and eliminate old ones. They use different names and standards for the new ones and never alert us to the fact that they are selling a new plan or an old plan under a new name. They may come out with a new plan in which we do not participate.
For example, we have a contract with insurance company ABC, under which we have seen you in the past, while you were participating in ABC’s “Green” plan. Next year, if your employer purchases the “Purple” plan to save money, the insurance company may say we do not have a contract for these same doctors under the “Purple” plan, making the doctors out of network. This makes the responsibility to pay the doctor to fall on you, the patient, rather than the insurance company. This is one example of how insurers use tricky tactics to make their customers pay while making it seem that these errors lie with the doctors when we are never notified of these types of changes.
It is CRITICAL that you check your network participation of the doctor you are seeing, prior to your appointment. Our offices and billing team, in spite of their best efforts, may not have access to these changes. Please do remember that all non-covered charges are your responsibility.
