Methods for obtaining your records free of charge:
1. Log in to your patient portal
- Hover over “Medical Records” in the left menu.
- Then click on visit summary to obtain physician notes and visit summaries.
- Click on current medications for a medication list
- Click on labs/ diagnostic reports for laboratory and imaging.
- You can download and pdf all of these documents.
2. Log in to the NYS Health Information Exchange
Paying for Medical Records:
In order to expedite the processing of medical records requests we have contracted with a third party to fulfill records requests.

FMLA, Disability and Other Forms
In order to expedite the processing of such forms requests we have contracted with a third party to fulfill records requests. Our charge in the past for this has been $40. RecordsQuests’s charges are lower.

If you are requesting pathology slides, that have originally been read by a Maiden Lane Medical pathologist, to go to another pathologist or laboratory for a second opinion or additional review, then please complete this form to start the process.
